After Denji realizes that his stomach is still connected to Pochita’s, he devours human trees in the Aging Devil’s realm, which leads to a fully-regenerated Black Chainsaw Man.

The chapter opens up with CSM bounding past a levelled city block to reach the Aging Devil, who offers a new contract with Public Safety to take down Chainsaw Man.

As soon as they agree, the Aging Devil rips out their nails, resulting in the Black Chainsaw Man being torn apart. The primordial foe plans to force-feed itself to Pochita, and the Public Safety agents ask to be sacrificed if it means preventing a prophecy of the age of devils.

Organ by organ, the agents are slowly pulled apart to prevent Chainsaw Man from reaching his destination. It slows him down some, so to the Aging Devil’s surprise, Pochita rips out his devil-heart and launches it directly towards the cabal’s headquarters.
